Holocrine secretion: the cell is the product

Sweat glands secrete across an intact membrane; sebaceous glands do not. They are holocrine, meaning the secreting cell dies to become the secretion. A sebocyte formed at the gland periphery moves inward while filling with lipid droplets, then its nucleus degenerates and the cell disintegrates entirely. The passage is commonly given as around a week.

The gland is a production line, not a reservoir with a tap, so what reaches the surface reflects cells that committed days ago. That is why nothing done to the surface changes production: cleansing, blotting and mattifying powders remove sebum already made and released, while the sebocytes behind them carry on.

Why hormone levels alone do not predict oiliness

Androgens are the dominant regulator, but the decisive conversion happens inside the gland. Sebocytes express 5-alpha-reductase type 1, converting testosterone into the more potent dihydrotestosterone, and can activate weaker adrenal precursors such as DHEA locally. The androgen concentration reaching the receptor is largely generated there, not simply delivered by blood.

This is why most people with oily skin have entirely normal circulating hormones: receptor density and sensitivity, and local enzyme activity, differ between individuals and between sites on one face. Output also follows a daily rhythm, highest around the middle of the day, and rises with skin temperature — commonly cited as roughly 10% per 1 °C.

The claim that stripped skin makes more oil

The idea that dehydrated skin overproduces oil to compensate is repeated constantly and is poorly supported. No feedback loop has been demonstrated between stratum corneum water content and sebaceous output, and the holocrine timescale argues against one: sebum arriving today was committed days earlier, not this morning.

Something real sits underneath the claim. Harsh surfactants strip intercellular lipids and irritate, so skin can feel tight while still looking shiny — the same quantity of sebum on a disrupted surface reads as greasier. Gentle cleansing is worth doing; it just does not turn the glands down.

Does oily skin need moisturizer?

Yes, sebum and hydration are not the same thing, and oily skin can still lack water in the upper layers. A lightweight, non-comedogenic moisturizer supports the barrier without adding heaviness.

Can washing my face reduce oil production?

Cleansing removes surface oil but does not lower how much sebum the glands make, and over-washing can strip the skin and prompt irritation. Gentle, consistent cleansing is generally more effective than aggressive degreasing.

Does sebum output really change through the day?

Yes. Measured secretion rates follow a circadian pattern, highest around the middle of the day and lowest in the early hours. It is one reason skin often looks shiniest by early afternoon.

Does sebum production fall with age?

It does. Output rises through puberty, holds through early adulthood, then declines gradually and more sharply after menopause as androgen support falls. That is part of why mature skin trends dry even in people once oily.

Is sebum the same as the skin’s barrier lipids?

No. Sebum is made in the gland and is largely triglycerides, wax esters and squalene, while stratum corneum barrier lipids are ceramides, cholesterol and free fatty acids made by keratinocytes. Plenty of sebum does not mean an intact barrier.

Does diet change how much sebum is made?

There is some human evidence that high-glycaemic diets, acting through insulin and IGF-1 signalling on the sebocyte, modestly increase output. The effect is small compared with genetics and androgen sensitivity.
