Androgens and Sebum Production — a group of hormones, including testosterone and its potent derivative DHT, that stimulate the sebaceous glands to produce sebum. Their activity largely determines how oily the skin is and plays a central role in the development of acne.

How androgens reach the sebaceous gland

Androgens arrive from the ovaries or testes and from the adrenal glands, which release the weaker precursors DHEA and androstenedione. The sebaceous gland is not a passive recipient: sebocytes carry their own enzymes, converting those precursors to testosterone and then, through 5-alpha-reductase, to dihydrotestosterone (DHT), which binds the androgen receptor considerably more strongly.

DHT binding switches on genes that drive sebocyte multiplication and lipid synthesis. The gland enlarges, its cells fill with oil as they mature, and at the end of their short life they rupture and release everything they contain into the follicular canal — a pattern called holocrine secretion. Androgens therefore set how many sebocytes there are and how much lipid each one makes, rather than opening a valve.

Why sensitivity matters more than circulating levels

Most people with persistently oily skin have androgen concentrations well inside the normal range. What differs is local: how much 5-alpha-reductase activity their glands have, how many androgen receptors the sebocytes carry, and how strongly those receptors respond once occupied. Oiliness is largely an end-organ trait, set at the gland rather than in the blood.

So a normal hormone panel does not mean hormones are uninvolved, and two people with identical results can have visibly different skin. Testing becomes more informative when oiliness appears alongside irregular or absent periods, new coarse hair growth or scalp thinning, which can point to conditions such as polycystic ovary syndrome. Interpreting that picture, and deciding whether to test at all, belongs with a clinician.

What changes it across a lifetime

Sebum output starts climbing at adrenarche, around the age of seven or eight, when adrenal androgens first rise. It accelerates through puberty, generally peaks in the late teens and early twenties, then holds fairly steady before declining with age — commonly earlier and more sharply in women after menopause, and more gradually in men.

Shorter-term shifts are smaller but real: secretion rises with skin temperature, so most people are oilier in heat and humidity, and some notice a modest premenstrual increase. Cleansing does not change what the gland makes — it removes sebum already on the surface, and the common claim that stripping the skin provokes compensatory oil production is poorly supported. What over-cleansing reliably does is disturb the barrier.

Why does my skin get oilier at certain times of the month?

Cyclical shifts in hormones, including androgens, can increase sebaceous gland activity, leading to more oil and occasional breakouts around specific phases of the menstrual cycle.

Are androgens only relevant to acne in teenagers?

No; androgen-driven sebum production can influence oiliness and breakouts at any age, and adult hormonal acne is common, particularly along the lower face and jawline.

Does washing more often reduce how much oil my skin makes?

No. Cleansing clears sebum that has already reached the surface, but it does not reach the gland or slow it. Frequent harsh washing tends to irritate the barrier rather than reduce oiliness.

Can a blood test explain why my skin is oily?

Usually not on its own, since oily skin is common in people whose androgen levels test entirely normal. Testing is more useful when oiliness appears alongside other changes, and that decision belongs with a clinician.

Do women produce androgens as well?

Yes. The ovaries and the adrenal glands both produce androgens, in smaller amounts than in men but more than enough to drive sebaceous activity throughout adult life.

Is oily skin the same thing as acne?

No. Sebum is one contributing factor, but acne also involves abnormal shedding inside the follicle, growth of Cutibacterium acnes and inflammation. Many people have oily skin and never develop acne; diagnosis and treatment belong with a clinician.
