Why the pattern falls where it does
The T-zone is a map rather than a behaviour. Sebaceous glands sit unevenly across the body — concentrated on the scalp, forehead, nose, chin, upper chest and back, sparse on the limbs. On the face the central band carries several times the gland density of the cheeks, which is why one person can be shiny at the nose and flaking at the jaw on the same morning.
Geometry adds to it. The forehead and the bridge of the nose are the most prominent forward-facing planes, so they catch direct light and the same film of sebum reads as shinier there. The nose also has the widest follicular openings on the face, making their contents visible as grey dots. Whether a zonal routine is worth the effort
Zonal routines are optional rather than necessary. A cleanser sits on the skin for well under a minute, so two different ones rarely repay the trouble. The change most people actually notice is two moisturisers — a fluid centrally, something richer on the cheeks — and that shows within about a week.
Actives are the other sensible place to be regional: apply salicylic acid or a retinoid where the congestion is and keep it off skin that does not need it. Judge salicylic acid at four to six weeks and a retinoid at eight to twelve. Escalating strength because an area is oily tends to backfire, since irritation spreads outward. When to see a clinician
A shiny central panel is not a medical finding, but some things appearing in the same territory are. Persistent redness with greasy yellowish scaling beside the nose, in the brows or at the hairline is commonly described as seborrhoeic dermatitis, which is managed rather than cured. Small papules and pustules ringing the nose and mouth, often after weeks of a potent topical steroid, are commonly described as perioral dermatitis and are assessed by a clinician.
Deep, tender lumps recurring along the jaw, or spots leaving pitted marks, deserve an early appointment, because the window in which scarring can be prevented is finite. So does a marked change in an adult who has never had an oily central face, particularly alongside irregular periods or new coarse hair growth. Anything on a child's skin belongs with a clinician.

Is an oily T-zone something that needs fixing?
Not in itself. It is the expected consequence of where sebaceous glands sit, and that sebum helps keep the surface flexible. Manage it only if it bothers you or comes with congestion.
Can the T-zone pattern change with age?
Yes. Output falls gradually from the late twenties and more noticeably after menopause, so the contrast between centre and cheeks usually softens. A forehead that shone at twenty often feels tight at fifty.
