The mechanism in full: where the water comes from
The intuitive model — that a moisturiser puts water into skin — is mostly wrong. Skin loses water outward continuously: the epidermis below is fully hydrated, the surface meets drier air, and water travels down that gradient. A moisturiser raises corneum water content by slowing that loss, so water moving up from below accumulates.
The rest follows. Occlusive and emollient lipids form the film that slows the loss, which is why measurable hydration appears within the first half hour. Humectants act inside the corneum: glycerol distributes into the tissue and behaves like natural moisturising factor. Why the effect runs on two timescales
The fast one is physical and reversible. Within half an hour corneum water content rises, corneocytes swell and fine roughness flattens, because a hydrated corneocyte sits flatter against its neighbours. That is the immediate glow — retained water, not delivered water. The film wears off over hours and hydration drifts back.
The slow one is biological. Repeated use keeps hydration higher, quietens the barrier's distress signalling and supplies lipids that can be built into the lamellar sheets — the source of improvements in baseline barrier function over one to several weeks. Stopping returns skin to its own baseline, not below it.
What the mechanism does not explain
Hydration instruments measure electrical conductance at the surface, not water molecules, and a glycerin-rich film can lift the reading by itself. A hydration curve describes the corneum plus whatever sits on it. Comfort is different: tightness owes as much to a dry corneum's stiffness as to its water content.
Two boundaries. No topical moisturiser meaningfully hydrates the dermis, so deep-hydration claims are not supported by this mechanism. And where dryness is persistent, fissured, itchy or inflamed, the cause may be eczema, ichthyosis or another diagnosable condition; moisturisers help manage those, but diagnosis and treatment belong with a clinician.

Applying moisturizer to slightly damp skin can help humectants trap the surface water and improve hydration. Sealing that moisture with the product then reduces how quickly it evaporates.
Do I still need moisturizer if my skin is oily?
Yes, hydration and oil are different, and oily skin can still be dehydrated in its upper layers. A lightweight, non-comedogenic formula supports the barrier without feeling greasy.
How long does one application actually keep working?
The occlusive part lasts as long as the film does — usually hours on a face that is not washed or rubbed. Humectants held in the corneum persist longer, which is why twice-daily use suits most skin.
Is a thicker cream always more hydrating than a lotion?
Not automatically. Thickness mostly reflects oil and wax content, which governs occlusion, but a lotion with a high humectant load can raise corneum water content as much while feeling lighter.
Does moisturising regularly make skin lazy or dependent?
There is no good evidence for that. Skin returns to its own baseline when you stop rather than dropping below it. What genuinely erodes the barrier is frequent hot washing and harsh cleansers.
Can a moisturiser repair a damaged barrier on its own?
It can support repair, and ceramides, cholesterol and fatty acids give the corneum material to work with over days. But the source of damage has to stop too. Skin that is broken or not improving over weeks warrants a clinician.
