What is actually happening

A pore is the surface opening of a hair follicle with a sebaceous gland attached. How large it looks is set by three separable things: the size and output of that gland, how well the follicular wall is supported by the collagen and elastin around it, and how much material sits in the opening.

Pores have no muscle and no sphincter, so they do not open or close. Hot water and steam do not open them; cold water and ice do not close them. Neither alters the diameter of the follicle, and neither changes how much sebum the gland below is making.

What genuinely reduces the appearance

Retinoids have the most consistent evidence. They speed follicular turnover so the plug stays smaller, and over months they build collagen around the follicle so the wall is better supported. Judge at 12 weeks. Salicylic acid 0.5–2% works on the plug rather than the wall, so it acts within weeks and reverses when stopped.

Sunscreen is preventive rather than corrective: it will not narrow a pore, but it slows the elastin loss that widens the wall. Astringent toners and vigorous scrubbing achieve nothing lasting. Prescription topicals and in-clinic devices are decided and delivered by a clinician.

When to involve a clinician

Pore size alone is cosmetic. It is worth an appointment when the same pores repeatedly become red and tender, when acne alongside them is leaving marks or indented scars — the window for preventing permanent scarring is finite — or when small yellowish bumps with a central dimple appear, commonly described as sebaceous hyperplasia.

Ask sooner about a single opening that keeps refilling with a firm core, or one that is enlarging, discharging, painful or hot. Any lesion that grows, bleeds, crusts, changes colour or fails to heal needs prompt review, and anything on a child's skin belongs with a clinician.

Cumulative sun exposure degrades the collagen and elastin supporting the follicular wall, so the opening loses its scaffolding. Pores widened this way are often oval and follow the pattern of sun exposure rather than oiliness.

Can squeezing the contents out make a pore smaller?

Emptying a pore makes it less visible for a few days, because you have removed what filled it rather than shrunk the follicle. Forceful squeezing can rupture the follicular wall, which risks inflammation and a wider opening.
