What is actually happening in the skin
The skin's mechanical strength sits in the dermis, in collagen bundles and a sparse elastin network. From the mid-twenties fibroblasts make less collagen and repair it more slowly, and elastin is barely replaced in adult skin. Ultraviolet light raises the activity of matrix metalloproteinases, the enzymes that cleave both proteins; glycation cross-links what remains, leaving collagen stiffer and slower to recoil.
Lines then behave in three ways. Dynamic lines are skin folding over a contracting muscle, and vanish as the face relaxes. Static lines are the same creases once the dermis beneath has thinned enough that the fold no longer springs back. Gravitational folds differ again: fat pads shrink and descend and the bone beneath remodels, so a jowl is a problem of volume and support rather than of the skin surface.
What genuinely helps, and how long it takes
The single most effective intervention is daily broad-spectrum sun protection, and it is preventive rather than corrective: it slows further photoageing but does not rebuild what has gone. Topical retinoids are the best-evidenced ingredient for fine wrinkling and texture, acting on turnover and dermal matrix production; allow twelve weeks at minimum, often six months, before judging.
Much of what is marketed here runs ahead of its evidence: peptide claims are largely extrapolated from laboratory work, collagen in a cream is too large a molecule to reach the dermis, and no topical resolves a gravitational fold. Resurfacing lasers, microneedling, injectable neuromodulators and fillers do more, but all are clinician-administered and carry real risks. None of it is obligatory.
When to see a clinician
The thresholds worth knowing have little to do with the lines. A spot within or beside a wrinkle that changes size, shape or colour, bleeds, crusts, or has not healed in a few weeks should be assessed. Rough, scaly, persistent patches on habitually exposed skin — scalp, ears, forehead, backs of the hands — warrant a clinical opinion rather than an exfoliant.
Sudden facial asymmetry, one brow that no longer creases, or new one-sided weakness needs same-day assessment, and with slurred speech or arm weakness it is an emergency. Prescription retinoids and in-clinic procedures are prescribed and monitored by clinicians, so a consultation is the place to weigh what is realistic for your skin.
