How a nail is made
Cells in the matrix divide, flatten and fill with hard keratins cross-linked by sulphur-containing bonds, which is what makes a nail rigid rather than flexible like the stratum corneum. Those cells die as they harden, so the visible plate is dead tissue throughout. It slides forward along the bed, which supplies the vessels seen through it as pink but adds nothing to its substance.
Because the plate is already dead, nothing applied to its surface changes how it grows — quality is decided in the matrix, and injury there shows up later as the affected segment emerges. What surface care does change is water content. Flexibility follows the water a nail holds, so repeated wetting and drying, detergents and solvents leave nails brittle and prone to splitting.
How quickly nails grow
Fingernails advance around 3 mm a month, so replacing a whole fingernail takes roughly four to six months. Toenails are slower, closer to 1 mm a month, and a great toenail can take a year or more. Growth tends to be faster in childhood and young adulthood, in warmer months, and on the dominant hand.
It slows with age, with reduced circulation, with immobility of the digit, and during serious illness. The practical consequence is a lag: what you see on the nail today was formed in the matrix weeks or months ago. A fever may leave a transverse groove that becomes visible only once it has travelled clear of the cuticle.
What surface changes do and do not indicate
Nails are often treated as a diagnostic readout, and they are not a reliable one. Fine lengthwise ridging is very common and more pronounced with age; on its own it is a normal ageing change rather than a deficiency sign. Small white marks usually follow minor knocks to the matrix and grow out. Brittleness most often reflects repeated wet-to-dry cycling and household chemicals.
Some findings deserve attention rather than interpretation at home: a new or widening dark band running the length of a nail, a nail lifting from its bed, thickening or crumbling, pitting, or changes with pain or swelling. Any of these is worth showing to a clinician, as is a change confined to one nail. The rule is not to match a sign to a diagnosis, but to have anything persistent looked at.
