The Dermis
The Dermis — the thick middle layer of skin beneath the epidermis, providing structural strength, elasticity, and nourishment. It is rich in collagen and elastin fibers produced by fibroblasts, and it houses blood vessels, nerves, hair follicles, and sweat and oil glands. This layer gives skin much of its firmness and resilience.

Two zones, not one
The dermis divides into two zones that differ in arrangement rather than in kind. The papillary dermis is the thin upper region, built from fine, loosely arranged collagen with a high proportion of ground substance. It rises into the papillae that interlock with the epidermis and carries the capillary loops that feed it.
Below it the reticular dermis makes up the bulk: dense, coarse collagen bundles lying largely parallel to the surface with elastic fibres woven through. Most of skin's tensile strength lives here, along with the follicles, glands, larger vessels and encapsulated nerve endings. The bundles follow lines of tension, which is why wound direction affects how a scar settles.
The matrix and the cells that maintain it
The dermis's mechanical character comes from the extracellular matrix, the material between the cells. Collagen supplies tensile strength and accounts for most of the layer by dry weight. Elastin is a small fraction but lets tissue return to shape after stretching. Between the fibres sits ground substance, a hydrated gel of glycosaminoglycans that binds water.
Fibroblasts build and maintain all of it, making collagen and elastin precursors and secreting the enzymes that break the old matrix down, in a turnover measured in years rather than days. Collagen applied to the surface does not reach the dermis: the molecule is far too large, and topically it acts as a humectant and film-former.
What wears it down
Cumulative ultraviolet exposure is the largest external influence. It raises matrix metalloproteinase activity, degrading collagen faster than fibroblasts replace it, and drives the build-up of abnormal clumped elastic material called solar elastosis. Sun-exposed sites show laxity and deep lines decades before covered ones.
Intrinsic ageing runs alongside: fibroblast activity declines and collagen production falls gradually from early adulthood. In women, dermal collagen loss accelerates around menopause as oestrogen falls. Smoking, chronically high blood glucose and long-running inflammation push the same way.

Both. Stretch marks begin as tears in the dermal matrix when skin is stretched faster than fibroblasts can remodel it. Scars are the dermis's repair tissue, laid down quickly in a more parallel arrangement than the original weave.
How much thicker is the dermis than the epidermis?
Considerably, commonly on the order of ten to forty times depending on site. The dermis is thickest on the back and thinnest on the eyelids, and accounts for most of the variation in overall skin thickness.
