What sweat actually does to the skin
Sweat is mostly water with salts and urea, and it comes from eccrine glands that open onto the surface rather than into the hair follicle. It is not delivered into the pore, and there is no good evidence that sweat is comedogenic. The trouble comes from what sweating creates: warm, wet skin held under fabric for hours.
Prolonged wetness swells the outer layer so that follicular openings block and inflame more readily, and friction adds mechanical irritation — which is why bumps trace the outline of a strap or waistband. Something separate happens in heat: the eccrine ducts themselves become obstructed and sweat leaks into the surrounding skin. What genuinely reduces it
Timing does most of the work. Getting out of damp kit and rinsing within roughly half an hour removes the occlusion while it still matters; a lukewarm rinse with a gentle cleanser is enough, and scrubbing is counterproductive. Loose, quick-drying fabric and less equipment pressure on reacting skin are the other two levers.
For bumps that persist between sessions rather than clearing in days, ordinary acne evidence applies: consistent topical treatment judged at eight to twelve weeks rather than at two, and body skin responds more slowly than the face. Miliaria needs nothing beyond cooling and drying and fades within days; acne actives tend to make it sting.
When to get it looked at
Bumps that come and go with activity rarely need medical attention. It is worth an appointment when lesions are painful, deep or leaving scars, since the window for preventing permanent scarring is limited; when they are itchy and strikingly uniform across the chest and upper back; or when nothing has changed after around three months of consistent care.
Some features need prompt review: spreading redness, fever or feeling unwell, or a lesion that is enlarging and increasingly painful, which suggest infection rather than blocked follicles. Heat rash with faintness, nausea or a temperature that will not settle points to heat illness rather than a skin problem and needs urgent care.
