Sun Spots and Solar Lentigines — flat, well-defined patches of increased pigment that develop on areas repeatedly exposed to the sun, such as the face, hands, shoulders, and upper chest. They form when ultraviolet light stimulates melanocytes to deposit extra melanin over time, and they tend to become more common with age. Unlike freckles, they usually do not fade in winter and persist year-round.

What decades of ultraviolet actually do

How pigment cells build melanin and hand it on is set out under dark spots. What defines a solar lentigo is dose: ultraviolet arriving in small instalments across decades until melanocytes in one patch both multiply locally and raise their output. The result is a fixed spot, not an even tan.

Hence the distribution — face, backs of the hands, forearms, upper chest and upper back — and hence their arrival from around forty. Their winter behaviour follows: a tan fades, while a lentigo reflects a settled local change and holds its colour year-round.

Prevention beats correction, by a wide margin

Nothing applied afterwards matches not accumulating the dose. Daily broad-spectrum sunscreen, reapplied on long days outdoors, with sleeves, a brim and shade at midday, keeps new spots from forming and old ones from deepening. Hands and chest are most often missed.

Correction is slower and partial. Topicals may soften the colour over three to six months but rarely clear a lentigo, because the pigment cell population itself has changed. Clinic procedures work more decisively, and are chosen after examination.

Spots that need a doctor, not a serum

Sun-damaged skin is also where skin cancers appear, so these sites deserve attention. Ask for an examination if a spot enlarges, changes shape or shade, develops an uneven border or several colours, becomes raised, itchy or scaly, bleeds, crusts, or fails to heal.

Rough, gritty patches that come and go on long-exposed skin are worth showing to a clinician, as is any spot that stands apart. A single new brown mark on skin that has never had many is a doctor’s question.

No; freckles are usually smaller, often hereditary, and tend to fade with reduced sun exposure, while solar lentigines are larger, have defined borders, and generally persist regardless of season.

Can sun spots be prevented?

Consistent daily use of broad-spectrum sunscreen along with protective clothing and shade can significantly reduce the formation of new sun spots, since they are largely caused by cumulative UV exposure.

Do sun spots ever go away on their own?

Rarely. Unlike a tan, a solar lentigo reflects a lasting local change in the pigment cells, so it tends to stay put. The colour may soften over a winter of low exposure, but the spot remains.

Why do sun spots appear on the hands first?

Hands are exposed constantly and protected least; sunscreen there is washed off through the day and seldom reapplied. Driving adds to it, which is why one hand or forearm is sometimes worse.

Can sunscreen fade spots I already have?

Not directly, but it stops them deepening and prevents new ones, which is why every fading routine is built on it. Without daily protection, actives spend their effort undoing fresh stimulation.

How long before I decide a product is not working?

Twelve weeks at minimum, and three to six months for a fair verdict. Sun spots respond more slowly than marks left by inflammation, and many never clear with topicals alone.
