Two production lines, not one
Skin makes lipids in two separate places, and most confusion about oil and dryness begins by treating them as one. The first is the sebaceous gland: whole cells inside it fill with lipid and disintegrate, releasing sebum into the follicle, from which it reaches the surface. Output is driven largely by androgens, climbing at puberty and falling later in life.
The second line runs inside the keratinocytes. As they mature through the granular layer they package lipid precursors into lamellar bodies, which are extruded at the boundary with the stratum corneum and converted there into the final mixture. Why oily skin can still have a damaged barrier
Because the two systems are independent, they can be out of step in either direction. Someone with abundant sebum can have a disorganised intercellular matrix and lose water rapidly, while someone with little surface oil can have a matrix in good order. A shiny surface tells you the glands are active, not that the wall behind them is sealed.
So the belief that oily skin cannot be barrier-damaged is wrong, and it pushes people towards stripping routines that worsen the underlying problem. Surfactants, alcohol and repeated exfoliation remove surface oil within seconds, which is visible, and deplete intercellular lipids gradually, which is not. What depletes each of them
Surface lipids come and go quickly: washing removes most of the film, and sebum resurfaces over the following hours in anyone with active glands. Intercellular lipids behave differently, because they are laid down once as cells mature. Restoring them means waiting for the next cohort of cells, so damage done over a weekend can take weeks to settle.
Both decline with age, on different schedules. Sebum output falls markedly after middle age and more sharply after menopause, while ceramide content in the outer layer also drops — a large part of why mature skin trends dry even where it was once oily. Cold air reduces barrier lipids seasonally, and ultraviolet exposure oxidises surface lipids such as squalene.

Are skin lipids the same across the body?
No. The face, scalp and upper trunk are rich in sebaceous lipids, while the lower legs have few sebaceous glands and rely almost entirely on the intercellular matrix — one reason shins are often first to feel dry.
Does washing with water alone preserve skin lipids?
Better than washing with surfactants, but not entirely. High temperature and prolonged contact still extract lipids and swell the outer layer, so long hot showers affect the barrier even without cleanser.
