Langerhans Cells

Langerhans Cells — specialized immune cells found within the epidermis that act as sentinels, detecting and responding to foreign substances, microbes, and allergens. They capture these invaders and help initiate the skin's immune response. As part of the body's defense network, they contribute to how skin recognizes and reacts to potential threats.

How the network samples the epidermis

Langerhans cells sit among the keratinocytes rather than in the bloodstream, held in place by adhesion molecules shared with their neighbours. They extend fine dendrites upward between cells, and these can reach through the tight junctions of the granular layer to sample material above without opening a gap in the barrier. They are identified by the surface molecule langerin and by Birbeck granules.

When a Langerhans cell takes up and processes antigen it changes state. It loosens its adhesion to surrounding keratinocytes, alters its receptor profile so that it responds to lymphatic signals, and travels through the dermis to a draining lymph node, where it presents fragments of what it collected to T cells. The epidermal network refills over the following days.

Ultraviolet light and local immune suppression

Ultraviolet radiation, particularly UVB, reduces both the number of Langerhans cells in exposed skin and the ability of those remaining to present antigen. Several mechanisms contribute: direct DNA damage, conversion of trans-urocanic acid in the stratum corneum to its cis form, and release of suppressive signals such as interleukin-10 by keratinocytes.

The consequence is a period of reduced local immune surveillance after significant sun exposure, generally days to weeks and longer with repetition. This is one reason ultraviolet damage is thought to affect skin cancer risk through more than mutation alone: it also dampens the response that might otherwise recognise abnormal cells. The same effect is used deliberately in supervised phototherapy.

Allergy, tolerance and an unsettled picture

Allergic contact dermatitis has two phases. In sensitisation, small reactive chemicals cross the stratum corneum and bind to skin proteins; Langerhans and dermal dendritic cells take up the complexes and prime T cells in the lymph node, with no visible rash. In elicitation, a later exposure recruits those primed T cells back into the skin.

The full role of Langerhans cells here is not settled. Experimental work indicates they can promote tolerance to harmless substances as well as drive activation, and that dermal populations carry much of the load in some models. They are better described as one node in a layered system than as a single switch. Their density also falls with age and chronic sun exposure.
