The mechanism in full
Water reaches the outer skin from below, diffusing down a gradient from roughly 70% water in the viable epidermis to about 15–30% at the surface. How much stays depends on natural moisturising factor — the amino acids, pyrrolidone carboxylic acid, lactate, urea and salts released as filaggrin is broken down in maturing corneocytes. NMF is hygroscopic and sits inside those cells, holding water within them. Applied humectants do the same from outside.
Moisturisation acts on the rate of loss instead. Ordered lamellae of ceramides, cholesterol and free fatty acids fill the space between corneocytes, and because those sheets are hydrophobic and tightly packed, water must detour around each flat cell to escape. Applied lipids help the lamellae reform, emollients fill gaps between loosened cells, and occlusives lower the vapour pressure gradient at the surface. None adds water: hydration fills the reservoir, moisturisation narrows the drain.
Why the two come apart in practice
Because the inputs are independent, so are the failures. Skin can hold an intact lipid barrier and still be short of NMF — after strong surfactants, in dry air, with age, or where filaggrin processing is reduced. Nothing stings, but underfilled corneocytes roughen the surface and scatter light unevenly, so the skin reads dull and finely crepey, and those lines flatten within minutes of a humectant.
The reverse pattern — adequate NMF, disorganised lipids — follows a retinoid ramp-up or over-exfoliation, and shows up as tightness, flaking, stinging and redness, because irritants now enter as readily as water leaves. The mechanism sets the timescales too: humectants change how skin feels within the hour, while lipid repair waits on keratinocytes making and secreting new ceramides, so recovery runs over days to weeks.
Where the two-category model runs out
The split is a teaching device rather than a clean biological boundary. Urea is a humectant at low percentages and, higher up, also loosens corneocyte adhesion and influences barrier lipid and antimicrobial peptide expression — supported, though the thresholds that matter in products are unsettled. Lactate is both an NMF component and a signal for ceramide synthesis, and glycerin's effect on aquaporin-3-mediated transport is supported rather than established for cosmetic use.
Two claims the mechanism does not support are worth naming. A topical humectant does not hydrate the dermis, where water depends on circulation and dermal glycosaminoglycans, and corneometers read only the outermost few micrometres. And dehydration is a description, not a diagnosis: tightness, flaking or persistent redness that does not settle with gentle cleansing and a lipid-containing moisturiser may be eczema or contact dermatitis, and a clinician should assess it.
