Where the smell actually comes from
Eccrine sweat — the watery fluid produced over most of the body — is essentially odourless: water, salt and a little urea. Apocrine glands, concentrated in the axillae and groin, secrete a thicker, lipid- and protein-rich fluid that is also odourless as it leaves the gland. The smell is made afterwards, on the skin.
Resident skin bacteria do the making. Corynebacterium species, and some staphylococci, metabolise apocrine precursors into volatile compounds — short-chain fatty acids and sulphur-containing thioalcohols — which is what the nose registers. That takes time, which is why fresh sweat smells of little and a worn shirt smells strongly, and why two people washing identically can smell different.
Antiperspirant and deodorant are not the same product
The split between the two is mechanistic, not marketing. An antiperspirant reduces the sweat reaching the surface, typically with aluminium salts that form a temporary plug in the duct. A deodorant does not reduce sweating at all; it acts on the bacteria, using antibacterial agents or acidifiers, or simply adds fragrance over the top.
Knowing which one you have explains a lot of disappointment. A deodorant will not keep a shirt dry, and an antiperspirant applied to already-damp skin performs poorly; most formulas suit dry skin at night better than a rushed morning. Washing, drying and changing clothes daily removes the substrate, but none of it changes the glands.
Odour that should be checked
Most body odour is ordinary biology, not a medical matter. What is worth an appointment is change: a distinct, sudden shift in your own smell that persists without an obvious cause. A sweetish or fruity odour, an ammoniacal or urine-like smell, or a strongly musty one can accompany metabolic, kidney or liver conditions.
Odour with other symptoms — fever, weight loss, unusual thirst, night sweats — should be assessed. So should malodour from a specific place rather than the body generally: a wound that smells, or a skin fold that is red, weeping or discharging. A sudden change in a child's body odour is assessed by a clinician.

Often modestly. Hair increases the surface on which apocrine secretion sits, so removing it can reduce how much odour builds through the day. It does not change what the glands produce.
Why do some people barely smell at all?
A common variant of the ABCC11 gene makes apocrine secretion dry and largely odourless. It is widespread in East Asian populations, and is the same variant that produces dry rather than sticky earwax.
