What it is made of
The hypodermis is mostly white adipose tissue: lobules of adipocytes, each cell almost entirely filled by one droplet of triglyceride, held in a scaffold of collagen bands called fibrous septa. The septa run from the underside of the dermis to the fascia over muscle, which is why skin slides across the body rather than shearing off it.
It is not inert padding. Adipocytes secrete signalling molecules — leptin, adiponectin and other adipokines — involved in appetite regulation, insulin sensitivity and inflammation, so subcutaneous fat is now generally described as endocrine tissue rather than simple storage.
How it shapes the face
Facial fat is not one continuous sheet. It sits in discrete compartments — deep and superficial midface pads, and temporal, periorbital and jawline pads — each with its own septal boundaries. They age unevenly: several of the deep pads lose volume while more superficial ones settle downward as their supports slacken.
The result is redistribution rather than simple loss: hollowing at the temple and under the eye, a flatter cheek, more weight along the jaw. Because this layer supplies most of the face's convexity, its decline tends to read as ageing more clearly than surface texture does. On the hands and shins the same thinning makes veins and tendons prominent.
Why skincare stops well above it
Nothing applied to the surface reaches the hypodermis in a meaningful amount. An ingredient would have to cross the stratum corneum, the rest of the epidermis and the whole depth of the dermis — one to three millimetres of tissue organised to keep molecules out — and most cosmetic ingredients stay in the outermost layer.
What does change this layer is systemic: energy balance, sex hormones, ageing, some medicines and illness. Where fat is lost or gained is largely set by genetics and hormones, not by treating one area. Prolonged potent corticosteroids can cause local atrophy; fillers and liposuction work only because they are delivered into the layer directly.
